• Please fill out the application (word document) below in it's entirety. 

• Applications must be completed by Monday, February 18th at 8pm.

• Applications must be emailed to: petsofsfah@gmail.com with the subject line AC/DC application. Please just attach the word document to the email. 

• Once we have received applications, we will proceed with the next phase of the adoption. This may include personal interviews, meet and greets, and home visits. 

• To ensure AC/DC has continued care, instead of an adoption fee,  we ask that he be enrolled in our monthly wellness plan. This wellness plan will include his neuter surgery which is not typical of our plans. The enrollment fee is $50.00 and then $46 per month. To read more about the wellness plan, click here.
